Questionnaires
Questionnaires are an invaluable tool that allows patients to take a proactive role in their treatment process.
Small, but important details can be forgotten during a patient’s description of their medical condition or relevant habits during an appointment with their physician.
However, by filling out a detailed questionnaire, patients are able to keep track of all relevant information, writing details down on an ongoing basis so that they are able to provide their physician with a better understanding of their condition. This information can then guide and improve a patient’s treatment based on the details provided.
For example, a headache diary may help to identify certain patterns or triggers; likewise, a post-procedure diary allows a physician to change or customize a treatment plan based on a patient’s diary information following a procedure. Take a small amount of time to make a big, positive difference in your health!
